Information of the General Financial Directorate on issuing Certificates of the Tax-payer´s Residence

This model application is intended only as an instruction, it is marked with a notice that it is a "specimen" as referred to Section 76 subsection 1 of Act No.280/2009 Coll., the Tax Code, as amended, according to which it is necessary to act with the tax administrator in the Czech language.

Therefore, it is not possible to submit the application in the English language, it is necessary to file the application in the Czech language.
 

Цей зразок заяви призначений лише як допомога при її заповненні та містить повідомлення про те, що це є “зразок“, із посиланням на ст. 76 п. 1 Закону № 280/2009 Збірки законів, Податкового кодексу із поправками,  згідно з яким з податковою інспекцію необхідно діяти на чеській мові.

Таким чином, заявку в україномовній версії не можна подати, а необхідно використовувати заявку чеською мовою.
